Following the implementation of the Commonwealth Home Support Programme (CHSP) in 2016 and the National Disability Insurance Scheme (NDIS) in 2018, Inner Gippsland HACC-PYP funded agencies are rebuilding their services to better meet the needs of HACC PYP consumers, their carers/families and advocates.
HACC-PYP is targeted at people aged under 65 and Aboriginal people aged under 50 who need assistance with daily activities, including personal care, dressing, preparing meals, house cleaning, property maintenance, community access and using public transport. Clients may require this assistance due to physical and/or psychosocial functional impairment related to disability (for which they do not meet NDIS eligibility requirements), chronic illness and short-term health needs, to remain living at home as independently as possible. (HACC PYP Program Manual 2013)
The Dept. of Families, Fairness and Housing (DFFH) funded a HACC-PYP Service Development Project Officer to work across the four Local Government Areas of Baw Baw, Latrobe City, Bass Coast and South Gippsland Shires, to support HACC-PYP funded agencies to better meet the needs of current and emerging HACC-PYP client cohorts in a changed funding environment. From the work the Project Officer undertook, the attached resources were developed and are available to download.
